How to treat macrocytic anemia?

A

325 mg (6.5 mg elemental iron) PO daily to TID. Accidental overdose o iron-containing products can cause fatal poisoning in children.. Side effects are constipation(dose related), dark and tarry stools. Can give docusate, antidote is desferal, deferoxamine.

Test dose, Ferumoxytol(Feraheme), Iron Sucrose(venofer). Trifereic added to the bicarb concentrate of hemodialysate

Low Hgb and HIGH MCV, Vitamin B12 and folate. IM or deep SC is 100-1000 mcg daily, weekly, monthly. Vitamin B12 comes in a nasal solution(Nascobal), 500mcg in one nostril weekly.

A

CKd is 3 x per week initiates when HgB <10. lower or interrupt when >11/ increased risk of death and thrombolytic events, watch for hypertension. Initiate in cancer for same parameters.

3 1/2 times longer t 1/2. weekly dosing, store them in fridge. IV route recommended for hemodialysis. Same side effects, dose IV or Sc weekly.

Beta0lactamase inhibitors, cephalosporins, isonazid, levodopa and methyldopa, penicillins, platniums, quinidine, quinine, ribavirin and rifampin. G6PD deficiency folks are chloroquine, dapsone, methylene blue, nitrofurantoin, primaquine, probenecid, rasburicase, sulfonamides.
