Middle cerebral a.
lateral surface of brain, basal ganglia, internal capsule
supplies arm/face area sensory/motor strip
Anterior cerebral a.
medial surface of brain, some lateral surface
supplies leg area sensory/motor strip
infarct –> contralateral weakness/sensory loss worse in LEGS
Posterior cerebral a.
medial surface of occipital lobe (*primary visual cortex and visual association cortex on medial surface. Also inferior surface of temporal lobes and thalamus)
infarct –> contralateral HEMIANOPIA
-bilateral –> cortical blindness (Anton’s)
lenticulostriate a.
main blood supply of internal capsule (fibers going to all areas - arm, leg, face)
ICA infarct
- Contralateral weakness
- Ipsilateral monocular vision loss (amaurosis fugax)
Anterior cerebral artery infarct
- Leg more than arm weakness/sensory deficit
- Language spared
Middle cerebral artery infarct
- Arm more than leg weakness/sensory deficit
- Aphasia on left, dysprosody on right
- Contralateral homonymous hemianopia/quadrantanopia
- Gaze deviation towards side of lesion
Posterior cerebral artery infarct
- Contralateral homonymous hemianopia
- Alexia without agraphia (left PCA involving splenium)
pure motor hemiplegia
internal capsule/ventral pons
pure hemisensory loss
thalamus
sensorimotor
thalamocapsular
Clumsy hand-dysarthria
Internal capsule, ventral pons, or corona radiata
Contralateral hemiballismus
SUBTHALAMIC NUCLEUS
