  • elderly (>60 yrs)
  • chronic onset
  • weakness: quads, wrist/finger flexors, neck flexors, dysphagia
  • associated with CTD (thyroid) but rarely malignancy
  • unresponsive to immunomodulation
  • characteristic RINGED VACUOLES
