Solar elastosis (actinic elastosis) is a common manifestation of chronic sun exposure and is a form of dermatoheliosis, or photoaging. It refers specifically to the histologic observation of massive deposits of amorphous, yellow, elastin-like (“elastotic”) material in the upper dermis that does not form functional elastic fibers.

Q

people at risk for solar elastosis

A

white people

Clinically, solar elastosis presents as yellow, thickened, coarsely wrinkled skin in a photodistribution. The skin can be both hyper and hypopigmented with scattered lentigines and flat seborrheic keratoses and flat pseudoscars on the forearms. Telangiectasias can be prominent, especially over the cheeks. Depending on the degree and amount of cumulative sun exposure, the findings can start to be observed in the third decade with gradual progression over the years. Fair-skinned individuals with a significant history of sun exposure are the highest risk group.

Favre and Racouchot disease (FRD), also known as solar or senile comedones, is a disorder of the skin resulting from chronic exposure to the sun.
The exact pathogenesis is unknown. After decades of actinic damage, small cysts and large blackheads form on the face and neck.

Q

difference between acne vulgaris and FRD/senile comedones

A

Unlike the comedones of acne vulgaris, comedones in FRD do not become inflamed.

Q

what activity puts people at higher risk of developing FRD

A

It is more common in patients with a heavy smoking history. Other types of radiation exposure also increase the risk. Males and whites are affected more commonly than females and darker-skinned individuals.
