Autonomic supply
- Sympathetic chain pelvic part (one on each side)
- Pelvic plexuses:
- Superior hypogastric plexus (presacral nerve)
- Right and left hypogastric (pelvic plexuses) - Parasympathetic supply (pelvic splanchnic nerve)
Sympathetic chain pelvic part course
- Abdominal part direct continuation
- Crosses sacrum ala deep to common iliac vessels
- It lies infront of sacrum anterior or medial to anterior sacral foramina
- Both chains unite in front of coccyx forming ganglion impar
Sympathetic chain pelvic part branches
Each trunk contains 4 or 5 sacral ganglia which give:
1. Lateral branches
2. Medial branches
Lateral branches
Grey rami communicants to sacral & coccygeal nerves
Medial branches
From first two ganglia to join inferior hypogastric plexus
Presacral nerve site
Below aortic bifurcation in front of L5 vertebra body
Presacral nerve formation
- Sympathetic fibers from aortic sympathetic plexus, 3rd and 4th lumbar splanchnic nerves
- Pelvic parasympathetic fibers
Presacral nerve branches
To:
1. Inferior mesenteric plexus
2. Uretic plexus
3. Gonadal plexus
4. Common iliac plexus
5. Also gives hypogastric nerves which share in inferior hypogastric plexuses formation
Pelvic plexuses site
Medial to internal iliac vessels
Pelvic plexuses branches
Supplies pelvic viscera
Formation
- Right and left hypogastric nerves (mainly sympathetic) from superior hypogastric plexus
- Branches from the first two ganglia of sympathetic chain pelvic part
- Parasympathetic fibers from pelvic splanchnic (S2, 3, 4)
Pelvic splanchnic nerve
- From sacral plexus (S2, 3, & 4)
- Parasympathetic ganglia are situated in pelvic organs walls
- Postganglionic fibers supply pelvic organs and hindgut
