II = well-controlled diabetes/HTN, smoker, obesity <40
III = severe systemic disease e.g. COPD, BMI >40,
IV = severe systemic disease that is a constant threat to life e.g. severe valve dysfunction, sepsis etc.
V = not expected to survive op e.g. ruptured AAA, massive bleed
VI = brain-dead
