Go through how you would explain inhaler technique

PSST

Previous knowledge + position
- explain to me what you know of your inhalers and how you currently take them
- get you sitting up nice and straight

Shake
- take the lid off the inhaler and make sure it’s clean and give it a good shake

Seal
- let out all your breath and make a tight seal around the inhaler - between your teeth but not biting down

Timing
- as you take a nice slow breath back in, press down on your inhaler and hold at the top for 10 secs

Then wait at least 30 secs before going again - if its the brown/steroid inhaler make sure to take a drink of water afterwards
Make sure to keep it on you so you have it when you feel breathless

If using a spacer - same rules apply but as you spray into the spacer (it acts as a container for the medicine) and you just need to take nice slow steady big breaths for 10 secs before repeating again
- option for face mask and normal

Make sure to clean the spacer according to instructions and drip dry rather than rub with anything as the static can cause medicine to stick to the sides

Remember 999 call for blue inhaler

How does inhaler technique change if it is a dry powder

A

As it is a dry powder important for a strong inhale

Twist it one way and then the other - make sure to listen for a click that means medicine is loaded in

Then everything the same except you just take a strong sharp inhale to get the medicine into your lungs

Genetic
- family short stature
- delayed growth or puberty
- genetic condition

Malnutrition - not uncommon in Tayside
Malabsoprtion

Acute illness can cause a weight drop but children should put the weight back on and this is nothing to be too concerned about at the moment - they are just using more energy than they normally would

Most important = multiple measurements and to see the trends - one measurement alone is not enough

Bloods if multiple low readings/other sx
