UNIT 11 Ethrypoetin Stimulating Agent Flashcards
Is Epoetin a replacement of blood transfusion
A. Yes
B. No
A. Yes
Action
Increases production of RBC’s within the bone marrow.(MOE HEMOGLOBIN)
Increases oxygen carrying blood cells
Uses
Treats anemia secondary to chronic renal failure or
chemotherapy
Prevent use of blood transfusion
Side effect of Epoetin?
Injection site reaction
Headache, muscle pain, joint pain
Adverse effect of Epoetin
Allergic reaction, mild-severe
Hypertension
MI, stroke
The patient’s Hemoglobin is 30 Is there a candidate for Epoetin?
A. No
B.Yes
A. No
you must monitor the patient HGB levels before administering Emotion, andministing Emotion with this patients HGB can cause blood clots or strokes or MI.
YOU NEVER ADMINISTER EPOETIN IF THE HEMOGLOBIN IS 11 OR MORE
What are the contraindications for Epoetin?
Contraindications
Uncontrolled hypertension
Allergy
Increased hematocrit
Caution with hemoglobin levels over 11 – at risk for
cardiovascular or thromboembolic events
Black box warning – if hemoglobin over 11, hx MI, stroke,
patient with cancer, increased risk of embolism have an
increased risk of death with this drug
Interactions
Androgens increase effect
can Epoetin cause hypertension , is this a normal finding?
A. Yes
B. No
B. No
Hypertention is an adverse effect
Nursing Intervention for Filgrastim and Epoetin
Nursing interventions
Monitor patient for side effects focusing on vitals signs, fluid
balance, DVT, cardiac, respiratory
Increase fluids to 2L day
Monitor for signs and symptoms of anemia (epoetin alfa) and
plan care accordingly. Monitor for SE/AR
Monitor for signs and symptoms of infection (filgrastim) and plan care accordingly. Monitor for SE/AR
Evaluation
Epoetin Alfa – increase in RBC’s
Filgrastim – No s/s of infection
How do you know that Epoetin worked?
increased in red blood cell
When working with a patient receiving epoetin alpha, it is
most important for the nurse to assess the patient for the
development of
a. bone pain and headaches.
b. hypertension and edema.
c. nausea and vomiting.
d. fluid retention and allergic reactions
b. hypertension and edema.
Before administering epoetin alpha, it is most important for
the nurse to assess the patient for a history of which
condition?
a. Anemia
b. Chronic renal failure
c. Pancreatitis
d. Hypertension
d. Hypertension
Answer: D
Rationale: Erythropoietin-stimulating agent administration is
contraindicated in patients with (1) uncontrolled hypertension, (2)
known hypersensitivity to mammalian cell–derived products, and
(3) known hypersensitivity to human albumin or polysorbate.
