take early in the morning, increase fluid intake, sit up right for an hour after administration to prevent esophageal reflux of esophagitis, used to prevent osteoporosis , do not use for pt’s with spine issues that cannot sit up
What is the use or action of Alendronate?
Action
Reduce the breakdown of bone microstructure –
the bone cells grow around the medication. Once covered by bone the drug is no longer active. It can take time to grow around the bone.
Uses
Prevention and treatment of osteoporosis
Can be ordered daily or weekly – weekly most commonly ordered
These have a very long half-life – for example alendronate can have up to a 10-year half life, so if the patient takes it
Side effect of Alendronate
Side effects
– GI distress
– Difficulty swallowing
– Esophageal reflux
These are not found in the injectable forms
of bisphosphonates, which are given
monthly, quarterly or yearly
Oral Alendronate Interactions
Interactions
– NSAIDS
– Aspirin
– Antacids
■ Contraindications
– Esophageal abnormalities
– Delayed gastric emptying
– Inability to sit or stand for 30 minutes after
ingestion
– Hypocalcemia – need calcium to build bone

– Maintain calcium and Vitamin D
supplements
– Teach to participate in weight bearing
exercise
– Prevention of falls
– Take medication weekly as ordered
– Take on empty stomach with 8 ounces of
water and sit upright for 30 minutes
– Wait 30 minutes after ingestion to eat, drink
or take other medications
