What causes subdural haemorrhage?

A

Rupture of bridging veins in the outermost meningeal layer usually due to trauma

How does acute SDH present?

A

Headache
Nausea/vomiting
Confusion
Drowsiness
Poor balance
Weakness
Paraesthesia or numbness

How is acute SDH managed?

A
  • CT shows hyperdense crescent collection not limited by sutures
  • Can cause midline shift/herniation
  • Managed with decompressive craniectomy

How does chronic SDH present?

A
  • More common in elderly/alcoholic patients
  • Week-Month history of progressive confusion/reduced consciousness/neurological deficit

How is chronic SDH managed?

A
  • CT shows hypodense crescent shaped collection
  • If asymptomatic can be managed conservatively
  • Otherwise surgical decompression with burr holes
