What is a subdural haemorrhage?
A
A collection of blood between the dura mater and the arachnoid mater

What causes subdural haemorrhage?
A
Rupture of bridging veins in the outermost meningeal layer usually due to trauma

How does acute SDH present?
A
Headache
Nausea/vomiting
Confusion
Drowsiness
Poor balance
Weakness
Paraesthesia or numbness

How is acute SDH managed?
A
- CT shows hyperdense crescent collection not limited by sutures
- Can cause midline shift/herniation
- Managed with decompressive craniectomy

How does chronic SDH present?
A
- More common in elderly/alcoholic patients
- Week-Month history of progressive confusion/reduced consciousness/neurological deficit

How is chronic SDH managed?
A
- CT shows hypodense crescent shaped collection
- If asymptomatic can be managed conservatively
- Otherwise surgical decompression with burr holes
