What is GBS?

A

An acute paralytic polyneuropathy which affects the peripheral nervous system, often triggered by Campylobacter/CMV/EBV infection

What is the pathophysiology of GBS?

A
  • B cells of immune system create antibodies against antigens on triggering pathogen
  • These match proteins on peripheral neurones
  • anti-GM1 antibodies in some patients

How does GBS present?

A
  • Symptoms present 2-4 weeks after infection
  • Progressive ascending weakness
  • Peripheral loss of sensation
  • Can have some back/neuropathic pain
  • Reduced reflexes

How is GBS investigated?

A
  • Nerve conduction studies
  • LP with CSF showing raised protein only

How is GBS managed?

A
  • Supportive care
  • VTE prophylaxis
  • IV Immunoglobulins/Plasmapheresis
  • Recovery can often take years
