A
aldosterone receptor antagonist so less Na absorption thru ENaC (more Na/water leaves) & less K excretion thru ROMK in principal cells of CD

Adverse Effects
A
Hyperkalaemia
Gynaecomastia (androgen effects to increase breast size)

Contraindications
A
Hyperkalaemia - use alpha/beta blockers for hypertension instead
Pregnancy - centrally acting (labetalol) decrease symp outflow
