prevention of thromboembolic events e.g. post MI, AF & stroke

Mechanism of Action
A
irreversible inhibits binding of ADP to P2Y2 receptor to inhibit activation of GPIIb/IIIa receptors (independent of COX pathway)
pro drug - active hepatic metabolites

Adverse Effects
A
bleeding!
GI upset- dyspepsia and diarrhoea
rarely- thrombocytopenia

Drug Interactions
A
CYP inhibitors e.g. omeprazole, ciprofloxacin, erythromycin & some SSRIs
caution with other antiplatelet/anticoagulant/NSAIDs (bleeding risk)
needs stopping 7 days prior surgery
might need other PPIs with use
