more efficacious & off patent compared to simvastatin
Indications
hyperlipidaemia
prevent CVS events
(higher dose for secondary prevention-CVS event)
Mechanism of Action
competitive inhibition of HMG-CoA reductase & upregulates hepatic LDLr & increased LDL clearance
REDUCE CVD risk by:
improve vascular endothelial function (more NO&VEGF/less endothelin)
stabilise atherosclerotic plaque (less SMC prolif & more collagen)
improve haemostasis (less fibrinogen&aggregation/more fibrinolysis)
anti-inflammatory (less CRP&adhesion&cytokines)
antioxidant (less superoxide form)
Adverse Effects
GI disrupt, nausea & headache
myalgia (as more CPK but dose related)
rarely rhabdomyolysis
increased liver enzymes
Contraindications
renal or hepatic impairment
pregnancy
breastfeeding
Drug Interactions
CYP 3A4
(amiodarone/diltiazem/macrolides/amlodipine/GRAPEFRUIT JUICE increase statin)
might need to withhold statin temporarily whilst other agents taken
