Common Indications
AF
secondary prevention of stroke/TIA/ACS
post primary PCI & stent
often with other antiplatelet agents
NSTEMI/STEMI - initial (once) 300mg loading dose chewable
acute ischaemic stroke - initial 300mg daily 2 weeks
Mechanism of Action
irreversibly inhibits COX1 mediated production of thromboxane A2 (potent platelet aggregator formed from arachidonic acid by CIX1)
action at low non analgesic doses (75mg) - higher inhibit endothelial prostacyclin PGI2
Adverse Effects
GI irritation/bleeding (peptic ulcer)
haemorrhage (stroke)
hypersensitivity
Contraindications
Reyes Syndrome (avoid under 16yrs)
hypersensitivity
3rd trimester can premature close ductus arteriosus
Drug Interactions
other antiplatelet & anticoagulant as additive/synergistic action
