What is the procedure for submitting and approving a change request to existing equipment or operational position layout?
A
Procedure:
Completion of Change Request Form:
- The Change Request Originator completes Part 1 and 2 of the Change Request form AA/FORM/186.
- Include STAR references if the change is related to a STAR Recommendation.
Peer Review:
- Submit the Change Request form to the TC for peer review.
- If the TC has no suggested changes, the form is forwarded to the GEM/GM for approval.
Assessment by GEM:
- The GEM, in consultation with appropriate Engineers, assesses the Change Request for practicality and performs a cost/benefit analysis.
- If the GEM agrees, the request is signed off and forwarded to the GM for final approval.
- ATC then processes the change, producing instructions and APSA as required.
- If the GEM disagrees, a statement explaining the reason for rejection is provided, and the request is forwarded to the GM for endorsement of rejection before returning to the Originator or TC.
Note:
If safety or business continuity is at risk, the GM or GEM can approve the change request immediately without further consultation.
