Classification for Pantoprazole
Antiulcer agent, and proton pump inhibitor
MOA (pharmacodynamics) of Pantoprazole
- Binds to the ATP pump in the basal gastric cell to prevent it from pumping protons (H+ ions) that will increase gastric acidity.
- Therapeutic effects: diminished accumulation of acid in the gastric lumen, with lessened acid reflux. Decreased acid secretion in hyper-secretory conditions
Indications for Pantoprazole Admin
- Erosive esophagitis associated with GERD
2. Upper and Lower GI Bleed (gastric and duodenal ulcers)
Dosage for Pantoprazole
IV: 40-80mg SIVP
Infusion: 8 mg/hr for 8 hrs. Mix 200 mg in 500 mL normal saline giving a concentration of 4 mg/mL.
Contraindications for Pantoprazole Admin
- Hypersensitivity to rabeprazole/related drugs (benzimidazoles)
- Pregnancy/nursing mother
Precautions in Pantoprazole Admin
- Caution in patients with severe liver impairment
- May increase serum levels of phenytoin and diazepam
Why are proton pump inhibitors (PPI’s), like Pantoprazole, indicated as first line therapy for upper/lower GI bleeds, erosive esophagitis, and GERD?
Because gastric acid can be a contributing factor to these conditions and PPI’s help reduce gastric acidity.
