Diltiazem Flashcards
Generic name for Cardizem?
Diltiazem
Trade name for diltiazem?
Cardizem
Classification - diltiazem
Class IV-Calcium Channel Blocker
What is diltiazem supplied in?
5 mg/ml (10 ml vial), 25 mg preload
Pharmacodynamics - diltiazem
❑ Antiarrhythmic:
- impedes the slow inward influx of calcium at the AV node thus decreasing conduction velocity and increasing the refractory period
- decreases A-fib or Aflutter
- decreases ventricular rate.
Indications (2) - diltiazem
❑ Tachy A-Fib or A-Flutter
❑ rentry SVT (with narrow QRS complex and adequate blood pressure) refractory to adenosine
Dose: first, second and maintenance
*I think this is Diltiazem?? Need to update card to be sure.
1st Dose: 0.25 mg/kg SIVP;
2nd Dose: 0.35 mg/kg SIVP;
-15 minutes after 1st dose
Maintenance Infusion: For acute rate control of A-Fib or atrial flutter maintenance is initiated at a rate of 5 to 15 mg/hr titrated to the patient’s heart rate.
Mix: 50mg in 500ml NS (concentration 0.1mg/ml)
Contraindications (8) - diltiazem
❑ Hypersensitivity ❑ Wide-QRS tachycardia of uncertain origin ❑ Poison-or drug-induced tachycardia. ❑ Sick Sinus Syndrome ❑ WPW ❑ Bradycardia; second or third degree AV block ❑ LV dysfunction; Heart Failure ❑ Hypotension
Precautions - diltiazem
❑ heart failure
❑impaired hepatic or renal function
❑ taking Beta-Blockers or Calcium Channel Blockers – use in extreme caution
❑ Not recommended for pediatrics
