MOA(pharmacodynamics) of Metoclopramide (2)
A
- Antiemetic: inhibits dopamine receptors in the brains chemoreceptor trigger zone to inhibit or reduce N/V. It blocks the D2 receptors that interact with the CTZ, and serotonin-3 receptors from interacting with the vagus nerve. It acts on vagus-4 receptors increasing peristalsis
- GI stimulant: Relieves esophageal reflux by increasing lower esophageal sphincter tone and reduces gastric stasis by stimulating upper GI tract, thus reducing gastric emptying time.

Indications for Admin of Metoclopramide
A
- Antiemetic (particularly with chemotherapy and postoperative)
- Gastroesophageal reflux, facilitation of radiographic examination
- Upper GI bleed, which will help with nausea and also help with pre intubation

Dosage for Metoclopramide
A
10 Mg IM/SIVP over 2 mins (single dose); may give 10 mg in 50 ML NS over 15 minutes. Repeat every 8hours.

Contraindications for Metoclopramide
A
- Hypersensitivity
- Pheochromocytoma
- Epilepsy
- Patients receiving drugs likely to cause extrapyramidal responses
- MAOI in the last 14 days
- Parkinson’s disease
- Bowel obstruction

Precautions When Administering Metoclopramide
A
- Hypertension
- depression
- NOT for pediatrics
- sedation and orthostatic hypotension
