Metoprolol Flashcards
Generic Name for Lopressor?
Metoprolol Tartrate
Trade name for Metoprolol?
Lopressor
Classification of Metoprolol
Beta Blocker/antihypertensive
How is Metoprolol supplied?
1 mg/mL in 5 mL ampules or preloads
Pharmacodynamics of Metoprolol
Selectively blocks beta 1 adrenergic receptors; slow heart rate, decreases cardiac output, decreases myocardial ischemia severity
Indications (3) for Metoprolol
❑ PO dose for patients with acute myocardial infarction (AMI) and no contraindications after the first 24 hrs of hospitalization
❑ Stable, narrow-complex tachycardias if rhythm remains uncontrolled or unconverted by adenosine or vagal maneuvers or if SVT is recurrent
❑ Control ventricular rate in patients with atrial fibrillation or atrial flutter
What is the dosage for metoprolol?
same for all 3 indications
Treatment of SVT (post-adenosine) / Atrial Fibrillation / Atrial Flutter: 5 mg SIVP (over 1 -2 minutes) doses q 5 minutes prn to a total of 15 mg; total dose should not exceed 15 mg (vital signs must remain stable), once stable 50mg PO
Contraindications (7) for Metoprolol
❑ Hypersensitivity
❑ CHF – severe LV failure
❑ Systolic blood pressure less then 100mmHg
❑ Cocaine use in last 24 hours with chest pain
❑ Heart block (greater than 1st degree or 1st degree with PRI > 0.24 secs)
❑ Bradycardia < 60 bpm
❑ Cardiogenic shock
Precautions (5) - Metoprolol
❑ Bronchial asthma ❑ Impaired hepatic function ❑ Impaired respiratory function ❑ Diabetes ❑ Not for Pediatrics
Notable notes to note - Metoprolol
❑ Concurrent IV administration with IV Ca Channel blockers can cause severe hypotension and bradycardia/heart block
