What is the MOA (pharmacodynamics) for octreotide

A
  • Suppresses secretions of serotonin, gastroenterohepatic peptides, insulin, glucagon and growth hormone.
  • Reduces secretion of fluids by intestine and pancreas.
  • Causes vasoconstriction and reduces portal vessel pressures in the bleeding varices.

A
  1. Emergency management of bleeding esophageal varices in patient with liver cirrhosis and for protection against re-bleeding
  2. Hypoglycemia secondary to oral hypoglycemic poisoning

Dosages for Octreotide admin (2)

A

Oral hypoglycemic OD refractory to dextrose:
Adult: 100 mcg IV/IO/SC q 6 hours prn
Peds: 2 mcg/kg/IV/IO/SC Q 6 hours prn

Upper GI bleed:
50 mcg IV/IO SIVP, then start infusion at 50 mcg/hr. Mix 500 mcg into a 250 mL NS which results in a concentration of 2 mcg/mL
