Octapeptide is a type of peptide that mimics a naturally occurring hormone known as somatostatin.
What is the MOA (pharmacodynamics) for octreotide
- Suppresses secretions of serotonin, gastroenterohepatic peptides, insulin, glucagon and growth hormone.
- Reduces secretion of fluids by intestine and pancreas.
- Causes vasoconstriction and reduces portal vessel pressures in the bleeding varices.
Indications for Octreotide administration
- Emergency management of bleeding esophageal varices in patient with liver cirrhosis and for protection against re-bleeding
- Hypoglycemia secondary to oral hypoglycemic poisoning
Dosages for Octreotide admin (2)
Oral hypoglycemic OD refractory to dextrose:
Adult: 100 mcg IV/IO/SC q 6 hours prn
Peds: 2 mcg/kg/IV/IO/SC Q 6 hours prn
Upper GI bleed:
50 mcg IV/IO SIVP, then start infusion at 50 mcg/hr. Mix 500 mcg into a 250 mL NS which results in a concentration of 2 mcg/mL

Hypersensitivity
Precautions for Octreotide
Consider lower doses for those with gallbladder disease or renal impairment.
Notable Notes for Octreotide
- When giving octreotide, monitor glucose frequently.
- Octreotide may enhance the toxic effects of drugs that prolong the QT interval.
