Classification - Levenox (enoxaparin sodium)
low-molecular-weight heparin (LMWH)
How is Levenox (enoxaparin sodium) supplied?
30mg/0.3ml ampoules; 30mg/0.3ml prefilled syringe
Actions (Pharmacodynamics) - Levenox (enoxaparin sodium)
❑ Anticoagulation action: LMWH that accelerates formation of antithrombin III-thrombin complex and deactivates thrombin preventing conversion of fibrinogen to fibrin.
Indications - Levenox (enoxaparin sodium)
Acute STEMI
Dosage - Levenox (enoxaparin sodium)
<75 yr: Initial dose: 30mg IVP; Second dose 1mg/kg SQ (15 minutes following initial dose), Note: maximum dose of 100mg SQ (Maximum SQ dose does NOT include IV dose)
> 75 yr: Eliminate initial IV dose; Give 0.75 mg/kg SQ q 12 hours, Note: maximum dose 75mg SQ
AHS MCP – As directed by VHR physician / protocol
Contraindications - Levenox (enoxaparin sodium)
❑ Hypersensitivity to heparin or pork products
❑ Active major bleeding or bleeding disorders
❑ Type II Heparin or LMWH induced thrombocytopenia
❑ Platelet count <100,000
Precautions - Levenox (enoxaparin sodium)
❑ Renal and hepatic impairment ❑ Pregnancy or lactation ❑ Patients using anticoagualants ❑ Uncontrolled HTN ❑ Recent surgery ❑ Renal insufficiency
Notes - Levenox (enoxaparin sodium)
❑ Not recommended for pediatrics
❑ Also used for prevention of DVT and treatment of acute DVT (1.5mg/kg/day or 1mg/kg twice/day SQ)
❑ Used for ACS patients with unstable angina/NSTEMI, however, creatine levels must be obtained
