Osteology (Part 1): Distal Tibia & Fibula Flashcards
Distal tibia articulates with
- Talus
Medial malleolus
- Subcutaneous
- Palpable
Medial malleolus structures
- Medial malleolar articular facet
- Anterior/posterior colliculi
- Intercollicular groove
- Groove for tibialis posterior and FDL tendons
Medial malleolar articular facet
- Pear shaped facet
- Lateral surface of medial malleolus
- Covered with articular (hyaline cartilage)
- Articulates with medial talar body
Anterior colliculus (tibia)
- Anterior projection
- Extends further inferiorly
- Ligament attachment
Posterior colliculus (tibia)
- Ligament attachment
Distal tibia structures
- Medial malleolus
- Tibial plafond
- Posterior process
- Fibular notch
Tibial plafond (tibia)
- Rectangle shaped, inferior articular surface
- Lined with articular cartilage
- Continuous with articular facet of medial malleolus
- Articulates with talus
Posterior process (tibia)
- Posterolateral prominence (3rd malleolus)
- Groove for flexor hallucis longus (depression over process)
Fibular notch (tibia)
- Triangular depression on lateral surface of tibia
- Articulates with fibula (tibiofibular syndesmosis)
Distal fibula
- Fibular shaft terminates at the level of tibial plafond
Distal fibula structures
- Lateral malleolus
- Lateral malleolar articular facet, fossa, and sulcus (fibular groove)
- Summit of lateral malleolus
- Medial triangular surface
Lateral malleolus
- Subcutaneous and palpable
- More distal and posterior than medial malleolus
- Resists eversion
Lateral malleolar articular facet
- Triangular facet on medial surface
- Articulates with talus
Lateral malleolar fossa
- Fossa posterior to articular facet (on medial surface)
- Ligament attachment (transverse tibfib and posterior talofibular)
Lateral malleolar sulcus (fibular groove)
- Groove on posterior border
- Fibularis longus and brevis tendons pass
Summit of lateral malleolus
- Rounded, distal end.
- Calcaneofibular ligament attaches
- Can be site of avulsion fracture
Medial triangular surface (fibula)
- Articulates with fibular notch of tibia
- Forms distal tibiofibular joint
- Syndesmotic joint, strong ligament
Talocrural joint
- Between tibial plafond, lateral malleolar articular facet, medial malleolar articular facet and talus
Midtarsal Joint (Chopart’s joint, transverse tarsal joint)
- Between rearfoot and midfoot
Lisfranc’s joint (tarsometatarsal joint)
- Between the midfoot and forefoot
Medial column of the foot
- Calcaneus
- Talus
- Navicular
- 3 cuneiforms
- Metatarsals 1-3
- Associated phalanges
- Sesamoid bones
Lateral column of the foot
- Calcaneus
- Cuboid
- Metatarsals 4,5
- Associated phalanges
Foot arches
- Articulation between bones and connecting ligaments
- Supported by intrinsic/extrinsic foot muscles/ligaments
- Shock absorption, support body weight
Medial longitudinal arch
- Higher than the lateral arch
- Calcaneus, talus, navicular, all cuneiforms, 1-3 metatarsals, sesamoids
- Calcaneus and heads of metatarsals 1-3 are the pillars
Lateral longitudinal arch
- Lower than the medial arch
- Calcaneus, cuboid, 4-5 metatarsals
- Calcaneus and heads of metatarsals 4,5 are the pillars
Transverse arch
- Formed by cuneiforms, cuboid, met bases
