Areas to highlight in a debrief
A
- Acknowledge and highlight successful outcomes and processes
- Provide the opportunity for personnel to discuss any issues or concerns, and capture relevant information in regards to operations
- Address any health and safety issues
- Initiate actions to resolve any significant issues
- Identify opportunities for improvement which may result in lessons to be learned for the organisation

Ensure the following occurs when conducting a debrief

- Avoid focusing on individual performance or issues that can be resolved at a local level
- Do not confuse opinions with observations
- Focus on the performance of systems and processes as a whole
- Capture key points and observations which may include successful outcomes and/or opportunities for improvement

Debrief checklist
A
- Organise time and suitable venue to cater for all participants
- Use FRV Debrief Template for discussion points
- Provide a diagram, maps and/or photographs of the incident site
- Introduce yourself and others
- Identify the aim of the debrief
- Set the ground rules
- Provide a brief overview of the background to the incident and include any pre-incident plans
- Provide a brief overview of the type of structure including any relevant building code requirements or deficiencies
- Review operations at the incident by facilitating input from participants in a chronological order of events. Provide a copy of the call history and any IAP (electronic or otherwise) and any available RPAS footage.
- Encourage input from participants and seek feedback
- Capture any observations but keep local/personal issues to discussion only. Record observations, insights and recommendations in the Debrief Template.
- Advise the participants that once finalised, the Debrief can be found under the Performance and Assurance Department tab on the FRV Intranet
- Thank all participants for their attendance
- Complete and send the FRV Debrief to the Performance and Assurance Department, the relevant ACFO Operations and the Incident Controller
