ORG 109 - OIC/Supervisor Responsibilities in the event of an incident, hazard or near miss
OICs/Supervisors are responsible for:
- Completing the FRVSafe initial report or ensuring that the reporting / injured / affected worker has completed the FRVSafe initial report within 24 hours of an incident occurring
- Completing an FRVSafe initial report on behalf of a visitor, contractor, subcontractor or member of the public who is affected by an incident / near miss / hazard
- Notifying their Commander or Manager if the incident is notifiable as per Appendix A or if the risk rating is rated ‘high’ or above

6.1 WHS Incident Management - Immediate Response
- Eliminate or isolate any remaining hazards at the incident site which may have the potential to cause any immediate further harm, if safe to do so
- Call 000 if necessary
- Notify supervisor / OIC
- Provide assistance as per 6.1.1 if an injury has occurred
- Supervisor/OIC must immediately notify the relevant Manager/Commander unless the hazard/incident is of a minor nature (eg. a facility issue requiring rectification)
*The Manager/Commander must be provided with information regarding the details of the injured or affected person, the facts of the incident and any medical transport and destination details which are relevant and - The Manager/Commander must notify the relevant ACFO/Director and relevant H&S Rep as soon as reasonably practicable
ORG 109 Appendix A - Incidents requiring notification to Worksafe
As per section 37 of the OH&S Act 2004, any incident that results in the following requires immediate notification to WorkSafe:
- The death of a person
- A person requiring medical treatment within 48 hours of exposure to a substance or
- A person requiring immediate treatment as an in-patient in a hospital
- A person requiring immediate medical treatment for:
*Amputation of any part of his/her body
*Serious head injury
*Serious eye injury
*Separation of his/her skin from an underlying tissue
*Electric Shock
*Spinal injury
*Loss of a bodily function
*Serious lacerations
- Any other injury to a person or other consequence prescribed by the regulations
- Any incident that exposes a person in the immediate vicinity to an immediate risk to the person’s health or safety through:
*collapse, overturning, failure or malfunction of, or damage to, any plant that the regulations prescribe must not be used unless the plant is licensed or registered
*The collapse or failure of an excavation or of any shoring supporting an excavation
*The collapse or partial collapse of all or part of a building or structure
*An implosion, explosion or fire
*The escape, spillage or leakage of any substance including dangerous goods
*The fall or release from a height of any plant, substance or object
*In relation to a mine:
**The overturning or collapse of any plant
**The inrush of water, mud or gas
**The interruption of the main system of ventilation
- Any other event or circumstance prescribed by the regulations
