Lidocaine: Class
Class 1B - Antiarhythmic/Antidysrhythmic, Anesthetic
Lidocaine MOA
Cardiac - Decreases automaticity by slowing the rate of spontaneous phase 4 depolarization.
Local Anesthetic: Inhibits transport of ions across the neuronal membrane, blocking conduction of normal nerve impulses.
Lidocaine Indications
Cardiac Arrest, VT/VF, VT w/pulse, PVC’s Preparalytic (RSI).
Local Anesthetic: For various procedures, including intubation and IO infusions.
Lidocaine Contraindications
Hypersensitivity, Hypotension, 2nd or 3rd degree AV blocks, Stokes-Adams syndrome
Lidocaine Adverse Reactions
Seizure, Slurred Speech, AMS, Anxiety, Confusion, Drowsiness, N/V, Tinnitus, Respiratory Arrest, Decreased HR, Bradycardia, Hypotension, Dysrhythmias, AV Block, Cardiac Arrest
Lidocaine NOTE
If pt is 70+yo, has liver dz, is in CHF or shock, reduce all doses, repeat doses & maintenance.
Lidocaine Dosage & Adm for Cardiac Arrest VT/VF
Adult : 1-1.5mg/kg IVP. May repeat twice w/0.5-0.75 mg/kg IVP Q 5-10 mins (Q 4 mins for ACLS) as an alternative to Amiodarone. Max dose 3mg/kg or 3 total doses.
Peds : 1mg/kg IV/IO (max dose 100mg)
Lidocaine Dosage & Adm for Stable VT / Maintenance Infusion:
1-4 mg/min. After a loading dose. (Mix 1g in 250ml of D5W)
Lidocaine Dosage & Adm for PVC’s
0.5-0.75mg/kg
Lidocaine Dosage & Adm for Local Anesthetic
Doses vary depending on procedure and location.
Duration of Action
Onset: 1-5 mins
Peak: 5-10 mins
Duration: Variable 15 mins-2hrs
