Class of Dopamine
Adrenergic, Vasopressor, Inotropic Agent
MOA of Dopamine
Produces positive inotropic & Chronotropic effects. Constricts systemic vasculature increasing BP & preload. Increases myocardial contractility & stroke volume.
Indications for Dopamine
Cardiogenic & septic shock. Hypotension w/low cardiac output states, distributive shock, second-line drug for symptomatic bradycardia.
Contraindications for Dopamine
Hypersensitivity, Hypovolemic shock, pheochromocytoma, tachydysrhythmias, VF. Decrease dose to 1/10th for pts taking MAOI’s.
Adverse reactions for Dopamine
Anxiety, AMI, chest pain, dyspnea, Ischemia, Palpitations, Increases Myocardial oxygen demand, tachydysrhythmias, HA, HTN, NV, TF, VT.
Special Note for Dopamine
Dopamine is ALWAYS given as an IV drip
Mix 400mg in 250ml of D5W & run at appropriate rate
Dosage of Dopamine for Adults
Doses greater than 20mcg/mg/min. Alpha effect predominate & may compromise circulation in the limbs.
Adult Renal Dose: 2-5mcg/kg/min IV/IO
Adult Cardiac Dose: 5-10mg/kg/min IV/IO
Adult Pressor Dose: 10-20mcg/kg/min IVO
Dosage of Dopamine for Peds
IV/IO infusion at 5-20mcg/kg/min SLOWLY titrated to pt response.
Duration of Action for Dopamine
Onset: 1-4 mins
Peak: 5-10 mins
Duration: Effects cease almost immediately after infusion is DC’d.
