If upon inspection of a city owned or operated facility an A-8 is forwarded to the agency having jurisdiction, forward a copy of the A-8 to the Public Buildings Unit, Bureau of Fire Prevention with the notation “City Agency” marked in RED in the upper right hand corner of the A-8.
Preparation and Disposition
1.2.1 Referral Report, A-8 shall be prepared in duplicate.
A. One copy shall be forwarded to the Bureau of Fire Prevention’s Field/Public Operations Support Unit (FPOSU). The second copy will be filed in the related “Building Record Folder.”
B. Referral items for DOB will be sent directly from FPOSU via a “green screen” (DOB) computer, which ensures prompt entries into the system. All final dispositions will be obtained and accessed via the web site of the department/agency of record through the FDNY Homepage on the intranet. FPOSU is no longer notified of final dispositions from the Department of Buildings and most other agencies.

1.3.1 The electronic A-8 form can be accessed on the FDNY intranet homepage under
“Forms”.
1.3.2 When the A-8 report contains referrals to FDNY or other city departments or agencies, all items shall be recorded on the same form. The letter in the item number indicates the department having primary jurisdiction.

F - Fire Department
B - Department of Buildings
1.3.3 Chief Officer endorsements are required for the A-8.

- Item 3B – Building: Vacant, Open, and Unguarded: Requests to seal a vacant building now must originate with DOB and then go to HPD. We must verify that the building is completely vacant. When the structural stability of the building is questionable a High Priority Referral Report may also be necessary.
HIGH PRIORITY REFERRAL REPORT 2.1 Purpose
The purpose of this program is to provide a procedure for notifying the Department of Buildings (DOB) when structural hazards or other serious life safety concerns with respect to matters under the jurisdiction of DOB are found by FDNY units. It ensures a prompt DOB and/or FDNY inspection and provides follow-up information to all units via the DOB website. It further ensures that progress on violations is monitored and that CIDS cards are prepared to identify the hazard to responding units. BATTALION CHIEFS MAY REQUEST A JOINT FDNY/DOB INSPECTION IF DEEMED NECESSARY..
Procedures
The procedures described herein should not be confused with, or be used as a substitute for, situations REQUIRING AN IMMEDIATE ACTION to be taken. Hazardous conditions relative to structural deficiencies/hazards or inadequate means of egress that are imminently perilous to firefighters or occupants shall be addressed utilizing existing procedures. Vacate procedures shall be initiated by the administrative units, or DOB, as conditions warrant…
A. When a serious hazard is discovered which necessitates the forwarding of a High Priority Referral Report to DOB, the following procedures shall apply:
• The administrative unit officer shall request the response of the administrative battalion for an evaluation of conditions.
• If conditions justify, the administrative unit shall complete the High Priority Referral Report found on the intranet for submission to the administrative battalion.
• The administrative unit will complete and forward a CIDS card.
B. The administrative battalion chief forwarding the High Priority Referral Report shall: Use a separate High Priority Referral Report for:
• Each address or premises
• Each case/enforcement work order for the same address or premises
• Take one or two digital photos documenting the condition. Photos are mandatory whenever the condition can be captured in this way. Always forward photos to DOB.
Ensure that the exact location of the hazard can be determined from the information provided. The endorsement box can be used to describe the location using FDNY exposure terminology.
Forward the High Priority Referral Report and photo(s) as an e-mail to the administrative division.
-Check the appropriate box on the Code 5A line and enter a separate case number in the designated box if a joint inspection is requested. Describe the reason for a joint inspection request in the endorsement box…
The DOB generated case number MUST be entered in the appropriate location on the report. The report and the photos shall then be saved in a file on the division computer. A hardcopy of the report, including the case umber, is forwarded to the administrative unit.

Inspections and Re-Inspections
• Within 24-36 hours of the receipt of our High Priority Referral Report, DOB anticipates conducting an inspection with their personnel. The DOB inspection status will be communicated through FDDB group, and inspection resolution will be emailed back to the FDNY caller. DOB representative at OEM will decide if it is a high-priority referral or an incident. If a joint inspection was requested, it will be conducted within a maximum of 20 days.
-Administrative units receiving notification that a reported hazard has been abated must conduct a re-inspection of the premises. If necessary, the CIDS information should be amended or deleted…

- Joint Inspection (Code 5A) - The Battalion Chief can request a FDNY/DOB joint inspection which will be scheduled and conducted within 20 days (priority B). The administrative division shall schedule the Joint Inspection with DOB by calling DOB Watch Command at 1718-422-8704 as soon as the field conditions / observations need to be addressed. HIGH PRIORITY REFERRAL Clarifications
The following guidelines are set forth below to ensure a more efficient use of the High Priority Referral Report. It is intended to clarify instructions on specific referrals:

Use this referral item when the structural stability of an existing building is in doubt. Examples on the exterior of the building include: large cracks in bearing walls, displacement of window sills/lintels, doorways out of plumb, cracks at corner of building, etc. On the interior look for sagging floors or stairs, cracks in walls/ceilings, windows dislodged, rust in steel beams, cracks or exposed rebar in concrete floors, cracks in interior walls corresponding to outside cracks, etc.

Use this item when the structural stability of a new building under construction is in doubt. Examples include: free standing walls/columns of excessive height, excessive storage of materials especially over metal decks, building leaning, cracks in foundation, etc.

Code 5B
The use of lightweight metal and wood construction materials has expanded tremendously over the last 10-20 years, chiefly because of their low cost. They can be found in new construction as well alterations. Because of their early collapse potential in a fire, the FDNY and DOB have been proactive in formulating new rules for the installation and inspection of these materials. DOB forwards a Monthly Structural Integrity Report to all Borough Commands listing all buildings in their districts which utilize one of these three construction elements. Administrative units are required to inspect the building and forward a CIDS card. This item is a high priority referral because these violations must be brought quickly to the attention of DOB before walls and ceilings, which can cover the violation, are installed. Requirements for each of these three buildings systems can be found as follows:
1. Laminated Wood I-Beams – Details can be found on our intranet at the DOB website under Technical Policy and Procedure Notice 8/92 (TPPN 8/92). These requirements are also summarized in chapter 5, reference 14.6.
2. Light Gauge Cold-Formed Steel & Metal Plate Connected Parallel-Chord Wood Floor Trusses – Notification or discovery of these two types of lightweight construction should prompt and immediate inspection, updating of building cards and the prompt submission of a CIDS card.

Vertical enlargements, the construction of additional stories on top of existing buildings, can present significant firefighting challenges for field units. The specifics of the requirements relative to these buildings can be found in TPPN #1/2000 on the DOB website. Forward this item if: the CONCEALED SPACE BETWEEN THE ORIGINAL ROOF AND THE NEW FLOORS ABOVE IS NOT FIRESTOPPED/SPRINKLERED; THE EXTINGUISHING SYSTEM SERVING THE ENLARGEMENT DOES NOT HAVE A SEPARATE WATER SUPPLY; THE ENLARGEMENT WAS NOT APPROVED BY DOB, ETC.
