The procedures in 1.1 and 1.2 do not apply to complaints related to places of public assembly such as theaters, sports arenas, convention halls, dance halls, cabarets or similar occupancies. Such complaints will be immediately telephoned to the Administrative Battalion Chief. The Battalion Chief will investigate with the Fire company and inform the Deputy Chief of actions taken, first by telephone and then in a written report. The Deputy Chief has the discretion to conduct an investigation if required.
Fire Prevention complaints which are received by Fire Prevention Operations Support Unit (FPOSU) that require a field unit response will be called into the appropriate borough communication supervisor. A request will be made that an administrative message be sent to the administrative unit to investigate the complaint. The complaint will include the address, nature of complaint, specific location or address and FP complaint number. This procedure will ensure that the unit receives the message ASAP whether located in quarters or in the field. The unit is responsible to investigate the complaint and forward either an interim or final Investigation of a Complaint Report (A- 17) within 24 hours of receipt of the complaint. The officer of the unit investigating the complaint must retain the administrative message as it will contain all the information required to complete the A-17 prior to faxing to FPOSU upon return to quarters.
Units are reminded that complaints must be investigated within 24 hours after receipt. Units receiving such complaints must make a diligent attempt to complete the inspection and each attempt should be recorded on the A-17. If access is not possible after several attempts within 24 hours from receipt of complaint, the check box shall be marked INTERIM REPORT in upper right hand corner and the A-17 shall be faxed to FPOSU indicating all attempts made and times. Additional attempts shall be made to complete the inspection within 72 hours. When completed, fax an A-17 FINAL REPORT to FPOSU. If the inspection cannot be completed, a telephone notification shall be made to the FPOSU after the 72 hour time limit.

The A-17 is to be prepared in triplicate. The original copy is forwarded to the Field/ Public Operations Support unit of the Bureau of Fire Prevention by the Officer conducting the investigation immediately upon return to quarters after conducting the investigation of the complaint.

- The Officer investigating a complaint from the Field/ Public Operations Support unit shall fax an unendorsed copy of the A-17 to 718-999-7108 or 718-999-0096 after conducting an investigation.
- Fire Department Referral Report, A-8, and/or special reports being forwarded as a result of the investigation shall be faxed with the unendorsed A-17 then attached to the original A-17 and forwarded for Battalion and Division review and endorsement.
All written complaints received through the U.S. Mail shall be faxed to the Field/Public Operations Support unit upon receipt, investigated and the unendorsed A-17 faxed to FPOSU after investigation. The written complaint shall be attached to the A-17 and forwarded thru the Battalion and Division for review and endorsement to the Field/Public Operations Support unit, Bureau of Fire Prevention.
Summons papers (A-200 etc.) or N.O.V. papers shall be placed in an envelope along with an unendorsed copy of the A-17 and forwarded directly to the Field Public Operation Support Unit, Bureau of Fire Prevention without delay. If any A-8’s and/or special reports were additionally generated as a result of the investigation, they shall be attached to the original A-17 and forwarded thru the Battalion and Division to Field Public Operation Support Unit.
