What is Cauda Equina syndrome?
Serious condition in which the lumbosacral nerve roots that extend below the spinal cord are compressed
What is the most common cause of Cauda Equina Syndrome?
Herniated disc/ Disc prolapse
What are other causes of cauda equina syndrome?
- Malignancy
- Trauma
- Infection
How does cauda equina syndrome present?
Saddle anaesthesia Severe back pain Sciatica like pain down one or both legs Sexual dysfunction Bladder and anal sphincter disturbance Unable to stand without assistance
What is saddle anaesthesia?
Reduced peri-anal sensation
Where would you get back pain with cauda equina syndrome?
Lower back pain
How is cauda equina syndrome diagnosed?
MRI
What would you get due to urinary retention?
A palpable bladder
What is the immediate management of cauda equina syndrome?
Steroids
What is the longer term management of cauda equina syndrome?
Urgent surgical decompression
When should surgical decompression be carried out with cauda equina syndrome?
Within 48 hours
What is a differential diagnosis of cauda equina syndrome?
Prostate cancer
What is a late sign that might indicate irreversible damage?
Urinary incontinence
Where does the disc prolapse tend to occur?
at L4/5 or L5/S1
What can late diagnosis of cauda equina result in?
Long term leg weakness and urinary/bowel incontinence.
