Pre-existing or known metabolic acidosis Acidosis refractory to effective ventilation Salicylate overdose Tricyclic antidepressant overdose Hyperkalemia Prolonged cardiac arrest Crush syndrome
Contraindication for sodium bicarbonate
None in the emergency setting
Adverse reactions to sodium bicarbonate
Extravasation may cause tissue necrosis
Routes of administration for sodium bicarbonate
IV
IO
Onset/Peak/Duration for sodium bicarbonate
O: Immediate
P: < 15 minutes
D: 1-2 hours
Adult dosages for sodium bicarbonate
1 mEq/kg
May repeat the dose 0.5 mEq/kg every 10 minutes
Pediatric dosages for sodium bicarbonate
1 mEq/kg
May repeat the dose 0.5 mEq/kg every 10 minutes
Special considerations for sodium bicarbonate
Must be used in conjunction with effective ventilation
