IV/IO: 0.05-0.1 mg/kg
IM: 0.2 mg/kg

Special consideration for midazolam
Consider decreased dosages for elderly patients and patients with liver and kidney disease due to slower metabolic function

O: 2-5 minutes
P: 3-5 minutes
D: < 2 hours

O: 15-20 minutes
P: 30-60 minutes
