- buffer excess buildup of hydrogen ions
- ↑ pH levels
- Provides vascular bicarbonate to assist the buffer system in reducing the effect of metabolic acidosis and in the Rx of some OD
Bicarb: Indications
- Metabolic acidosis from circulatory insufficiency from shock of severe dehydration
- ASA OD
- Prolonged cardiac arrest
- Tricyclic OD
- Known hyperkalemia
- Crush injuries
- Phenobarbital OD
Bicarb: Contraindications
Metabolic or respiratory alkalosis, electrolyte imbalance from vomiting or diarrhea, known hypokalemia
Bicarb: S. E.
Extravasation of IV may cause chemical cellulits with tissue necrosis. Gastric distention, gas, hypokalemia, alkalosis.
Bicarb: How supplied
Prefilled syringe 8.4% solution (50-mEq/50mL)
Bicarb: Adult dose
1 mEq/kg IV/IO repeat dose q 10m (8.4% solution)
Bicarb: Pedi dose
1 mEq/kg IV/IO
- Use 4.2% solution for infants
Bicarb: Precaution
Not recommended for routine use in cardiac arrest. Inactivates nor-epi and dopamine; forms a precipitate with calcium. Use caution with elderly with renal/cardiac failure with or without CHF. PRN in tricyclic OD until QRS narrows
