Amiodarone: Actions
Blocks sodium channels at rapid pacing frequencies. Delays repolarization and ↑ duration of action potential. After IV use, it relaxes vascular and smooth muscle, reduces peripheral vascular resistance (afterload). Slows HR because it blocks sodium channels
Amiodarone: Indications
VF and pulseless VT after defib. Stable VT and unstable VT after cardioversion
Amiodarone: Contraindications
AV blocks, hypersensitivity, cardiogenic shock, bradycardia
Amiodarone: S.E.
Cough and progressive dyspnea. Worsening of arrhythmias, symptomatic bradycardia, sinus arrest, SA node dysfunction, CHF edema, hypotension, cardiac arrest, HA, N/V, abd pain, shock
Amiodarone: Supplied
50mg/mL vials and prefilled syringes
Amiodarone: Adult Dose
Pulseless VF/VT: 300mg IVP, consider repeating once 150mg IVP in 3-5m.
Wide complex tachycardia (stable): 150 mg rapid infusion IV (15mg/min) over first 10 minutes.
May repeat 150mg rapid infusion IV q 10m PRN
Amiodarone: Ped Dose
Pulseless VF/VT: 5mg/kg IV/IO bolus.
Perfusing VT: 5mg/kg IV/IO over 20-60m
Amiodarone: Precautions
Pregnancy safety: Cat. D.
May produce vasodilation and hypotension. May have - inotropic effects May prolong QT interval.
