Lidocaine: action
↓ automaticity by slowing phase four of depolarization. ↑ VF threshold
Lidocaine: indications
Cardiac arrest from VF/VT. Alternative to amiodarone. Prevents rise in ICP associated with administration of succinylcholine and intubation procedure.
Lidocaine: contraindications
Hypersensitivity. 2nd or 3rd degree AVB with wide complexes, wide complex escape beats with bradycardia
Lidocaine: S. E.
Anaphylaxis, bradycardia, hypotension, cardiovascular collapse, seizures, malignant hyperthermia, respiratory depression, tremors, lightheadedness, confusion, tinnitus, blurred vision or double vision, and vomiting
Lidocaine: adult dose
VF/VT: 1-1.5mg/kg repeated q 5m.
Following doses at 1/2 of original, max of 3mg/kg.
After suppression of ectopy or dysrhythmia, initiate an infusion at 2-4 mg/minute
Lidocaine: pedi dose
VF, pulseless VT, wide tachycardia (w/pulses) 1 mg/kg IV/IO loading dose
Lidocaine: precautions
Discontinue infusion if signs of toxicity. Potentiates amiodarone beta-adrenergic blockers (inderal), and Tagamet. Toxicity can occur due to reduced metabolism of lidocaine in pts with liver disease
