Adenosine: Action
Naturally occurring nucleoside that acts on the AV node to slow conduction and inhibit reentry pathways. Rapidly metabolized– Half-life is 5 seconds
Adenosine: Indications
To convert stable narrow tachycardia (SVT) to normal sinus rhythm. Includes SVT associated with accessory bypassed tracts (Wolff–Parkinson–White syndrome). Not effective in converting rapid AF/Atrial flutter. May be helpful in clarifying borderline wide complex rhythms as dosage will not work on VT
Adenosine: Contraindications
Hypersensitivity. 2nd or 3rd degree heart block, sick sinus syndrome, or symptomatic bradycardia. Poison or drug induced tachycardia. COPD pts
Adenosine: Adult dose
Initial dose: 6mg IV or IO rapid bolus over 1-3 seconds. Followed quickly by 20mL saline flush; Elevate extremity.
Repeat 12mg in 1-2 minutes if needed.
Third dose: 12mg, maybe given in 1-2 minutes.
Adenosine: Pedi dose
Initial dose: 0.1 mg/kg (max of 6mg) rapid IV or IO followed by 5-10mL flush of NS
Second dose is 0.2mg/kg, max of 12mg with 5-10mL flush
Adenosine: S. E.
Generally short duration and mild. Persistent arrhythmias and bronchospasm
Adenosine: Considerations
Could produce bronchoconstriction in patients with asthma. Will produce short-lived asystole
