mechanism of digoxin
A
causes vagus nerve to increase release of acetylcholine which then slows HR to decrease constant neuronal firing in atria/cardiac workload
also blocks NaKATPase pump to increase contractile force/decrease cardiac workload

issues w/digoxin
A
eliminates symtpoms of heart failure but doesn’t protect against cardiac remodeling
NTI: 0.5-0.8 (ATI), 0.5-2 (union)
interactions: diuretics, ACE inhibitors, ARBs, antacids
hold if apical HR<60 for 60 sec; MUST listen, not palp
