The reverse side of the Emergency Operation Arrest Form will be displayed by the officer while being photographed with the prisoner. An index fingerprint of the officer and the subject will be affixed to the rear of the photograph. The photograph will be stapled to the Emergency Operation Arrest Form. Except for the officer’s copy, all other copies will be forwarded with the prisoner to a holding facility.
INFO:
Should unusual circumstances exist requiring detainment for more than four hours, arrangements to provide a meal are required. No more than ___ hours should elapse between meals. Detainees shall receive ___ meals in any 24-hour period.

*Radio transmitters, including base stations, portable radios, and cellular telephones, within 300 feet of the affected site, will be turned off until authority to transmit has been granted.
Officers encountering suspected car/truck bombs shall establish a perimeter of at least __ feet in all directions from the vehicle; e.g., two or three streets or avenues in each direction from the suspect vehicle. Adjacent buildings, vehicles, and civilians within the perimeter shall be completely evacuated. Command posts, if established, must be situated outside the safe distance perimeter. Adjustments to the size of the perimeter shall be made only after consulting with the Bomb Disposal Unit.

*In the absence of a suspected or actual explosive device, the decision to search and/or evacuate any bomb threat scene is the responsibility of the owner or person responsible for the facility.
While working in a radiation area, personnel will monitor their personal CDV-138 dosimeter every __ minutes and record their exposure on the Radiation Exposure Record. Upon being relieved, employees will return dosimeter units, the Radiation Exposure Record, and other issued equipment to PIEB personnel. The PIEB will forward collected radiation exposure data to the MDPD Safety Officer.

CDV-138 Low Range Dosimeter (0-200 mr): The CDV-138 is a self-reading dosimeter that measures radiation and can be read by an individual at any time. The dose that is read off the dosimeter is the total amount of radiation received. These are worn at ___ level, outside of the clothing.
