inadequate breathing (respiratory failure)
- some breathing but not enough to sustain life
- diminished or absent breath sounds
- noises like strider, snoring, gurgling, or gasping
- cyanotic skin color
- decreased minute volume
- O2 saturation below 95% (low oxygen)
intervention
Assisted ventilation (forced into lungs)
- pocket mask
- bvm
- FROPVD
not breathing at all (respiratory arrest)
intervention -Artificial ventilations - pocket mask -bvm FROPVD ATV 10-12 (we say 15) adult 20/min for child (oxygen powered devices are not used on children or infants because of dangerous high pressure to their airways)
Adult breaths adequate
12-20 bp/min
child breaths adequate
15-30 bp/min
infant breaths adequate
25-50 bp/min
Positive pressure ventilation dangers
decreasing cardiac output (blood pressure)
gastric distention
hyperventilation - the danger of blowing off too much carbon dioxide is that it may cause vasoconstriction and further blocking blood from reaching the brain.
Techniques of Artificial ventilation
- mouth to mask
- 2 rescuer BVM (preferably with oxygen at 15 liters per min)
- flow restricted, oxygen powered ventilation device
- 1 rescuer BVM (preferably with oxygen at 15 liters per min)
Patients who are vomitting/ or have vommitus in airway
DO NOT VENTILATE!!
-could cause vomit to be forced into lungs
signs of inadequate ventilation
- chest doesn’t rise and fall equally
- rate of ventilation is too fast or too slow
CPAP/BiPAP non invasive positive pressure ventilation
continuous positive airway pressure
biphasic continuous airway pressure
-any NPPV can only be used on patients who can breath on their own. it simply follows the breathing rhythm of the person and assists with higher concentration of O2.
FROPVD
flow restricted, oxygen powered ventilation device
- a device that uses oxygen under pressure to deliver artificial ventilations. its trigger is placed so that a rescuer can operate it while still using both hands to maintain seal on face mask.
- has an automatic oxygen delivery flow restriction to maintain proper delivery of oxygen to patient
Automatic transport ventilator (ATV)
- device that provides positive pressure ventilations. it includes settings designed to adjust ventilation rate and volume. while being portable and easy to carry on ambulance.
- ideal for 1 rescuer applications
