What bp med should be avoided in stroke b/c might increase pressure in the head?
Nitroprusside
Hypotension, abdominal pain, pulsatile mass is what?
Ruptured AAA
Screening for AAA?
All men 65 - 75 who smoked
Men less than 5.5 or women less than 5 check every 6 months
Stanford A aortic dissection?
A for including any ascending. B is descending
Main risk factor for aortic dissection?
HTN IS NUMBER 1, trauma, coarct, syph, ehlers danlos, marfan
Imaging for suspected dissection?
CXR, you see widening
Then get CT with contrast for better look
Dissection tx?
Beta blocker to decrease impulse!
Stanford A requires emergency repair (makes sense b/c branches get messed up)
Stanford B can be treated by Beta blocker (think B for Beta Blocker). Unless ruptures, or can’t control bp, or ischemia
How do you know if severe claudication?
ABI
Meds for PAD?
Gluc and BP control
Cilostazol (arterial dilator that can’t be used if CHF)
Daily aspirin or clop
STATINS
What do you do before surgery for claudication?
Do a stress test first b/c atherosclerosis could be all over
DVT tx?
Elevate
LMWH heparain
Then Warfarin
IVC filter if contraindicated for warfarin (like people at high risk of falling)
