Approach to atopic dermatitis Flashcards
What are the three main causes of allergic skin disease in dogs?
- Atopy
- Flea allergic Dermatitis
- Food allergy
What is atopic dermatitis?
Genetically predisposed inflammatory and pruritic allergic skin disease with characteristic clinical features associated with IgE, most commonly directed against environmental allergens
- impaired barrier function
- igE mediated hypersensitivity reaction
- Environmental alergens involved in pathogenesis
- Atopic-Like Dermatitis
What dog breeds are predisposed to atopic dermatitis?
Shar-Pei, Boston Terrier, Cocker Spaniel, Pug
What are the clinical signs of atopic dermatitis?
- Pruritus
- Erythema
- Saliva staining
- Recurrent otitis
- Pyoderma
- Pododermatitis
- Malassezia dermatitis
- Excoriation, Lichenification and alopecia
How would ypu diagnose food based dermatitis?
Diet trial
* Novel protein and carbohydrate source
* 6-8 weeks
* If recurrence following reintroduction of food then positive diagnosis
How does intradermal testing work?
- Mast cell reactivity due to presence of IgE
- Allergens should be chosen appropriate to the region
- Performed on the lateral thorax
How does igE serology work?
- Often done on a panel of allergens
- Vraiation between tests is used
How would you manage flare factors?
- Topical or systemic antibiotics
- Itraconazole for Malassezia
- Parasite control
How would you manage skin and coat care?
- Emollient shampoos
- Frequency and intensity of bathing is important
What is allergeb-specific management?
- Allergen avoidance
- Consider pollen/ house dust mites
- Consider where the animal is exercised
What would you use for pharmacotherapy?
- Topical glucocorticoid
- Oral glucocorticoid
- Oclacitinab
How would you manage flare factors?
- Parasite control
- Use oral/ systemic
- House dust mites
- Reconsider diet trials
What pharmacotherapy would you use for chronic atopic dermatitis?
- Glucocorticoids
- Oclacitinib
- Ciclosporin
