Chlamydia symptomen
MAN- Uretritis, procitis
VROUW - Veranderde fluor, contactbloedingen, buikpijn, dyspareunie, cervicitis, uretritis, proctitis
70-95% V en > 50% M asymptomatisch (Ct infectie)
25% asymptomatisch (LG infectie)
–> zo opbouw van onopgemerkt reservoir
Chlamydia complicaties
MAN - epididymis
VROUW - PID, infertiliteit, EUG, tubair ovarieel abces
M & V - SARA (sexually acquired reactive artritis)
Chlamydia diagnostiek
MAN - NAAT/ PCR 1e straals urine
VROUW - NAAT/ PCR via vaginale selfswab
Receptief anale seks < 6 mnd/ anale klachten = NAAT/ PCR proctum
Chlamydia behandeling
- Trachomatis infectie
- 1: azitromycine (1x)
- 2: doxycycline (2dd, 7d) - Trachomatis infectie rectaal
- 1: doxycycline (2dd, 7d) - Trachomatis infectie zwanger
- 1: azitromycine (1x)
- 2: amoxicilline (3dd, 7d) - LGV = lymphogranuloma venereum
- 1: doxycycline (2dd, 21d)
- 2: erytromycine (4dd, 21d)

Gonorroe symptomen

30-60% V en > 10% M asymptomatisch
Anorectale gonorroe: vaak asymptomatisch, irritatie, jeuk, afscheiding
Gonorroe complicaties
MAN - epididymis, prostatitis, chronische infectie (uretrale stricturen)
VROUW - bartholinitis, PID/ tubair ovarieel abces, EUG
Gonorroe diagnostiek
MAN - NAAT/ PCR 1e straals urine, kweek urethra uitsrijk
VROUW - NAAT/ PCR via vaginale selfswab
Receptief anale seks < 6 mnd/ anale klachten/ MSM = NAAT/ PCR proctum
NAAT/ PCR, grampreparaat (snel diagnostiek M), kweek + resistentie bepaling
Gonorroe behandeling
- Ceftriaxon (1x) (+ azitromycine (1x) bij chlam)
- Ciprofloxacine (1x)
- Amoxicilline (1x)
Gedissemineerde gonorroe
- Ceftriaxon (iv, 7d)

Herpes symtpomen
Soms asymptomatisch
Erosies, blaasjes met helder vocht > indrogen en korstvorming
Prodromaal soms jeuk of branderigheid

Herpes diagnostiek
NAAT HSV ½ op materiaal uit blaasje/ erosie/ korst
Herpes behandeling
- Valaciclovir (2dd, 5-10d)
- Bij recidiverend > onderhoudstherapie 1dd
Hepatitis B verwekker

Syfilis verwekker
- Bacterieel (spirochreet)
- Treponema Pallidum
- Seksueel contact, moeder-kind
- Incubatie tijd gem 3 weken tot primaire laesie, 6w – 12m tot secundaire stadium
Syfilis symptomen
Stadium I = pijnloos ulcus, lymfklierzwelling
Stadium II = roseolen, exantheem, haaruitval, wratachtige afwijkingen (condylomata lata), algemene ziekteverschijnselen
Stadium III = gummata, neurosyfilis, vasculaire syfilis
Syfilis diagnostiek
Donkerveldmicroscopie, serologie, kliniek, EIA’s, CLIA’s (–> bevestigen met treponemale test = TPPA/ TPHA)
Non-treponemale test (RPR) volgt ter bepaling ziekteactiviteit
Syfilis behandeling
- Stadium I
- Bezathinebenzylpenciline (1x)
- Doxycycline (2dd, 2w) - Stadium II/III OF zwanger
- Bezathinebenzylpenciline (dag 1,8 en 15)
Syfilis partnerwaarschuwing
Stadium I = alle partners laatste 3m
Stadium II = alle partners laatste 6m
Controle: VDRL/ RPR en klinisch (neurologisch) follow up
Neurologische afwijkingen of geen goede reactie op therapie liquoronderzoek t.u.v. neurosyfilis
HIV verwekker
- Viraal
- Human immunodeficiëntie virus
- Seksueel contact, moeder-kind, bloed-bloed
- Window fase: tot 3mnd na laatste risico contact
HIV symptomen
50-80% griepachtige verschijnselen en/ of rash binnen 2-4 weken
AIDS: opportunistische infecties, neurologische ziektebeelden en neoplasieën

- Combitest serologie HIV-1 en HIV-2 + p24 antigeen
- Western blot (confirmatietest)
