What is goal of lockout/ tagout?
to achieve “Zero Energy State”
When is lockout/ tagout guideline to be followed?
whenever source of elec., pneuamatic, hydraulic, gas, water, steam, chem., mechanical energy may present safety hazard
What is lockout?
isolation of power and/ or securing control device to machinery, equipment, deivce or thing which will secure it on off position w/ locking device
What is locking/ control device?
a device that isolates machinery/ equipment from power source and can be locked off
What is tagout?
the use of DANGER- DO NOT OPERATE tags to forbid others from operatng tagged device
What is blanking off?
all lines, systems that may introduce hazmat into workplace shall be disconnected/ blanked off. Valves may leak, shutting off may not be adequate
Who should be summoned to de-energize equipment initially?
qualified personnel
What are considerations to prepare for shutdown? (3)
- amount and type of energy/ hazards
- method of controlling main energy source
- stored/ secondary energy that stays in system after shutdown (ex. batteries)
- may be more than one type of energy involved
What is to be done after switch is locked out?
attempt ot re-start machine using normal starting procedure.
Who shall remove lock afterwards?
ONLY personnel responsible tasked with lockout
Whom is notfied when lockout is complete?
IC and/ or Safety Officer
For elevator rescues, what is t be done if there is noone available to hand over to and room is locable, what is to be done?
remove locking device and replace with zip tie
What needs to be documented? (3)
- names of staff applying locks/ tags
- names of maintenance staff on scene
- anyone responsible for removing locks/ tags
Who is responsible to ensure all sources of energy are neutralized?
