What are features of dilated cardiomyopathy?
Most common form of cardiomyopathy
Dilation of all 4 cardiac chambers leading to systolic dysfunction
Caused by alcohol, chemotherapy, myocarditis, peripartum
CXR- balloon heart
3rd heart sound
What are features of Takotsubo cardiomyopathy?
Caused by severe emotional stress
Chest pain
ST elevation- normal coronary arteries on angio
What are features of restrictive cardiomyopathy?
Caused by amyloiudosis (myeloma)
Sarcoidosis, Haemocrhomatosis, Scleroderma
ECG- low voltage ECG, bundle branch blockW
What are features of atrial myxoma?
Cardiac tumour
Weight loss
Fever unknown origin
Clubbing
Fatigue
SoB
Mid diastolic murmur with tumour plop
ECHO- pedunculated mass
What is Lynch Syndrome?
HNPCC
Autsomal dominant mutation
90% develop cancer- colorectal/endometrial
Amerstamd criteria
- Colorectal in 3 family members across 2 generations with 1 <50 at diagnosis
What is the induction of labour management?
Bishop score <6 vaginal prostaglandins or misoprostol
Bishop score >6 amniotomy and IV oxytocin infusion
What antibodies are seen in drug induced lupus?
Anti histone antibodies
How is pregnancy associated hypertension diagnosed?
HTN <20 weeks= pre-existing HTN
HTN >20 weeks= pregnancy induced HTN
Pre-eclampsia= HTN and proteinuria
What are features of lithium overdose?
Tremor
Ataxia
Hyperreflexia
Nystagmus
AKI
Mild/moderate- IV 0.9% saline
Severe= haemodialysis
When can medical management of ectopic pregnancy take place?
No significant pain/haemodynamic compromise
Seruim hCG <1500
Adnexal mass <35mm
Can return for follow up
Use methotrexate
What are features of malingering?
Faking symptoms for some benefit
What is the management of MS?
DMDs are indicated if >2 attacks within 2 years and significant functional limitation e.g. unable to walk >100m
