What are features of myelofibrosis?
Onset approx age 65
Anaemia
Thrombocytopenia
Leukopenia
Massive splenomegaly
Blood film- tear drop poikilocytes
What is the management of Chronic Myeloid Leukaemia?
Imatinib
Other treatment- hydroxyurea, IFN-alpha, BM transplant
What contraceptives are unaffected by enzyme inducing medication?
Copper Coil
Mirena coil
Progestogen only injectable
What are features of drug induced Parkinsons?
Suspect if rapid onset and symmetrical symptoms
Akinesia- with loss of arm swing
Bradykinesia
Hypomimia- difficulty with speech
Tremor is less common
Treat with antimuscarinics- procyclidine, benzatropine
What are features of CNS Lymphoma?
Seen in those with HIV
Headache, fever confusion
CT head- solitary homogenously enhancing lesions, thallium spect positive
What is the management of subarachnoid haemorrhage?
Medical- Nimodipine to reduce vasospasm
Surgical- angiogram, coiling
What is acute paronychia?
Pain/swelling/erythema at nail base following injury
Due to staph aureus
If minor infection- topical fusidic acid
If extensive infection- PO flucloxacillin, I&D if collection
What is a feature of megaloblastic anaemia?
Hypersegmented neutrophils
What are features of amyotrophic lateral sclerosis?
UMN affecting lower limbs
LMN affecting upper limbs
What are features of primary lateral sclerosis?
Affects UMN of arms legs and face
What are features of progressive muscular atrophy?
Affects LMN only
What are features of progressive bulbar palsy?
Dysfunction of brainstem motor nuclei
Affects the muscles of mastication/swallowing and tongue
Worst prognosis
What are features of progressive supranucelear palsy?
Parkinsonism
Vertical gaze impairment- looking down
Postural instability and unsteady gait
What is the management of STI induced epididymo-orchitis?
Ceftraixone and Doxycycline
What is the management of campylobacter infection?
If severe or immunocompromised- clarithromycin
