What is PVD
Disorders that change the flow of blood through the arteries and veins
Decreases the amount of blood flowing
Atherosclerotic
loss of elasticity, calcification of arterial walls
Arteriosclerosis
When fat or cholesterol obstructs and hardens the arteries
Where does thrombosis occur
In the lower extremities more then upper ones
CVI
Trouble getting blood back, inadequate venous return over long period of time
Commonly caused by hypercoagulagte state: 10+ hours of sitting
Congestion
Risk factors to PVD’s
Smoking, Hypertension, High Cholesterol, DM, Family history, Overweight, No physical exercise
Etiology of PVD
Greater in adults older then 80 years old
Men affected more than women
African Americans have higher rates then Whites or hispanics
Prevention of PVD
Maintain a healthy lifestyle
Screen at risk px
Take preventative meds
PAD manifestations
Intermittent claudication! Pain due to decreased blood flow= hypoxic tissue
Rest pain
Decreased peripheral pulses
Thin, shiny, hairless skin
Thickened toenails
Skin breakdown
Edema
CVI manifestations
Lower extremity edema, pallor
Cyanotic= brownish color from breakdown of RBC!
Recurrent stasis ulcers
Management of PAD
Symptom relief
Promote adequate circulation
Lower legs, dangle, DO NOT elevate
Heal/Prevent tissue damage
Management of CVI
Reduce edema
Discourage prolonged sitting or standing
Wear compression hosiery!
Elevate legs and feet! want to assist with blood return
Treat stasis dermatitis and ulceration
PAD
Trouble getting blood out
Nonpharmacologic therapy
Smoking cessation!
Foot care
Rest when claudication starts and resume when pain resolves
Control: DM,HTN,Cholestrol and BMI
