What is Pneumonia and its types
An infection of the lungs
Can be caused from bacteria or virus
Can cause puss, WBC or difficulty of gas exchange
Types include: Cap;community acquired, Aspiration pneumonia, HAP:Hospital acquired, Ventilated acquired
What are the risk factors to Pneumonia
Compromised immune system
Immature immune system
Smoking or Diabetes
Clinical manifestations related to pneumonia
Dypsnnea, cough, chills, fever, cyanosis
Diagnostic testing to Pneumonia
Chest Xray, CBC, O2 sat, Bronchoscopy
Nursing interventions to Pneumonia
Monitor respiratory, Asculate lung sounds, Suction as needed, Monitor vitals, Skin color, Educate on incentive spirometer, Hydrate and Turn and position HOB at 30 degrees
Interpersonal treatment of pneumonia
Pharmacological, antibiotics if caused by bacteria
Providing oxygen
Speech Therapy
Virus; treat the symptoms
Increase fluid intake
Prevention of Pneumonia
Vaccinate with pneumoccal vaccine
PPSV23 for ages 65 and older
BUT, if immunocompromised= HIV, DM, sickle cell, anemia and malignancies, can get as early as 2 yrs old to 64.
RSV
Highly contagious, Droplet/Contact
Causes bronchioles
More common in daycare and children before age 2
Who is at risk for RSV
Infants and toddlers in daycare
Exposure to second hand smoke
Wood burning stoves
Older adults with chronic disease
Daycare workers
Clinical manifestations of RSV
Irritability, Cough, Low grade fever, Copious mucis secretion
Nursing interventions for RSV
Administer oxygen
Supportive care is best
No vaccine is available so , Proper hand hygiene
Levels of prioritization
First: ABC’s vitals and labs: Airway, Breathing
Second level: Mental status change
Untreated medical issues
Acute pain
Acute elimination
Delegation of LPN
Scope depends on state and facility
Can only care for stable patients, not acute
Helps collect data
Foley care
Wound care
Chronic patients too
LPN restrictions
No blood products, IV’s, Unstable patients, TAPE, New admissions, Discharge teachings, Fresh postop
Delegation CNA
Scope depends on state ad facility
ADL’s, Toileting, Bathing, Ambulate, Blood draw, Assist in feeding
