Narcotic (opioid) antagonist
Mechanism of Action
Competitive inhibition at narcotic receptor sites. Reverses respiratory depression secondary to narcotices, Completely inhibits the effect of Morphine.
Indications
ALOC patients with suspected/known opioid overdoses presenting with hypoventilation.respiratory arrest
Contraindications/Precautions
Hypersensitivity; use with caution in narcotic-dependent patients
Adverse effects
Withdrawal symptoms, especially in neonates (nausea, committing, diaphoreses, increased heart rate, falling blood pressure, tremors). Be prepared for combative patient after admin
Incompatabilites/ Drug interaction
Should not be mixed with other drugs
Routes of admin
Slow IV/IO push, IN, IM
Onset of Action
Onset in 2min, peak variable; duration 45min
Adult dose
0.01mg/kg, slow IVP/IN to total of 10mg
Pediatric dose
0.01 mg/kg IVP, if ineffective five 0.1 mg/kg IVP (if IV access is unobtainable, may administer Narcan IN)
Special considerations
Opioid drugs include: heroin, deluded, morphine, meperidine, codeine, methadone, lomotil, darvon, darvocet, talwin , and others. Large amounts of the drug may be needed for Darvon, darvocet, and methadone
Be prepared for Combative patients after admin
