Calcium Channel Blocker
Mechanism of Action
Inhibits calcium ion influx across the cell membrane
Indications
Hypertension in subarachnoid bleeds, Hypertensive urgency/crisis
Contraindications
Advanced aortic stenosis; caution in heart failure or significant LV dysfunction because of the negative inotropic effect; caution in beta blocker usage
Adverse effects
Hypotension, headache, nausea, and vomiting, tachyarrhythmia
Incompatibilities/Drug interactions
Possible with fentanyl and vecuronium in same line
Routs of admin
IV drip. Do not push
Onset of action
1-2 minutes; peak 2-7 minutes; duration of effects is 44 minutes
Adult dose
2.5mg/hr - 15 mg/hr IV: titrate in increments of 2.5 mg/hr every 15 minutes
Pediatric dose
Not approved
Special Considerations
Most common side effect is rapid drop in blood pressure: avoid hypotension in possible stroke patient; Continuous monitor blood pressure, heart rate and EKG
May produce a prolonged duration of effect in renal and hepatic patients.
