Class
Anti-dysrhythic, endogenous nucleoside
Mechanism of action
Slows conduction time through the AV node; can interrupt the reentry pathways of the AV node. Onset of action within 5 to 20 seconds following an IV dose;half-life is 10 seconds
Indications
To slow the rate of narrow complex tachycardia. He’s only effective on SVT and PSVT
Contraindications
Sick sinus syndrome, Second and 3rd° AV block, known atrial fib relation\flutter, WPW
Precautions
Has no effect on ventricular contractility, causes peripheral vasodilation
Adverse effects
Transient dysrhythmias (sisterly, bradycardia, PVCs), bronchospasm, facial flushing, metallic taste
Adult dose
Initial dose: 6 mg rapid IVP over 1–3 seconds followed by rapid 10 ml NS flush
Second dose: if no response within two minutes from initial dose 12 mg; rapid IVP with 10 mL NS flush.
Third dose: if no response within two minutes from second dose. 18 mg; rapid IVP with 10 mL NS flush.
Pediatric dose
Initial dose: 0.1 mg/kg rapid IVP/IO Over 1-3 seconds. Maximum initial dose 6 mg
Second: if no response within two minutes from initial dose 0.2 mg/kg rapid IVP/IO maximum second dose not to exceed 12 mg
